This section includes the forms utilized by Kentucky Office of Vocational Rehabilitation counselors to provide services to Kentuckians with disabilities. These forms are presented for informational purposes to consumers, however, we ask that you speak with a Kentucky Office of Vocational Rehabilitation counselor or staff member for technical assistance prior to completing and submitting any form. Please note that eligibility must be determined by the counselor and no service can be provided without authorization from a Kentucky Office of Vocational Rehabilitation counselor.
